What Is Ayushman Bharat PM-JAY?

Launched on 23rd September 2018 by Prime Minister Narendra Modi in Ranchi, Jharkhand, Ayushman Bharat Pradhan Mantri Jan Arogya Yojana is widely recognised as the world’s largest government-funded health insurance programme. It operates under the Ministry of Health and Family Welfare and is implemented by the National Health Authority (NHA).

The scheme provides a health cover of ₹5 lakh per family per year for secondary and tertiary care hospitalisation to over 12 crore poor and vulnerable families — covering approximately 55 crore beneficiaries, which is nearly 40% of India’s population.

Ayushman Bharat works on two key pillars:

  • Ayushman Arogya Mandirs (formerly Health & Wellness Centres): Over 1.78 lakh centres across India, providing free primary healthcare services closer to people’s homes, including general OPD, medicines, diagnostics, and teleconsultation.
  • Pradhan Mantri Jan Arogya Yojana (PMJAY): The insurance component covering hospitalisation costs at empanelled public and private hospitals across India.

 

Key Features of the Scheme

  1. Cashless and Paperless Treatment

Beneficiaries can walk into any of the 36,229 empanelled hospitals across India and avail of cashless, paperless treatment — no upfront payment, no mountain of forms. The Ayushman card is the only document needed.

  1. Coverage for Pre-Existing Diseases

Unlike many private insurance schemes, PMJAY covers all pre-existing conditions from Day 1. Diabetes, heart disease, kidney problems nothing is excluded right from the moment you enrol.

The scheme covers more than 1,900 medical procedures across 27 specialities, including:

  • Cancer treatment and chemotherapy
  • Heart surgeries and bypass procedures

There is no cap on the number of family members who can benefit. Elderly parents, children, and all members are covered equally.

Who Is Eligible?

Eligibility for PMJAY is determined based on the Socio-Economic Caste Census (SECC) 2011 data for rural and urban areas. Broadly, the following categories qualify:

  • Rural families with specific deprivation criteria — such as no pucca house, daily wage labourers, SC/ST households, or families with disabled members
  • Urban workers in occupations like rag picking, domestic work, construction, street vendors, and sanitation workers
  • All senior citizens aged 70 years and above — regardless of income — since September 2024
  • Frontline workers such as ASHA workers, Anganwadi workers, and their families (about 37 lakh families)
  • Gig workers (delivery agents, ride-hailing drivers, freelancers) — announced in Union Budget 2025, covering approximately 1 crore gig workers

Milestone Numbers: How Many Have Benefited?

  • Over 43.52 crore Ayushman cards have been issued
  • 14 crore ‘Vay Vandana’ cards issued exclusively for senior citizens aged 70+
  • More than 36,229 hospitals empanelled — both public and private
  • Families have saved over ₹1.52 lakh crore in out-of-pocket medical expenses
  • Over 8.19 crore people have received treatment under the scheme
  • Delhi became the 35th state/UT to implement the scheme in April 2025

Between FY 2015 and FY 2022, Government Health Expenditure (GHE) rose from 29% to 48% of total health spending, while Out-of-Pocket Expenditure fell from 62.6% to 39.4% — a direct impact of schemes like PM-JAY.

The scheme has grown significantly in the past two years:

Senior Citizens Get Special Coverage (October 2024)

The Union Cabinet approved health coverage for all individuals aged 70 and above, irrespective of their income or family status. These beneficiaries receive a dedicated annual cover of ₹5 lakh through the Vay Vandana Card. For senior citizens already enrolled under a PM-JAY family, they get an additional ₹5 lakh top-up.

Gig Workers Brought Under Coverage (Budget 2025)

The Union Budget 2025 announced that approximately 1 crore gig workers — including delivery executives, ride-sharing drivers, and platform-based freelancers — will be covered under PM-JAY. These workers will be identified through the e-Shram portal and receive ₹5 lakh annual health coverage.

Budget Allocation Increased by 29%

In Union Budget 2025–26, the government allocated ₹9,406 crore for AB PM-JAY — a nearly 29% jump from the previous year. The broader health budget stood at ₹99,859 crore, underlining the government’s commitment to universal healthcare.

Special Card Drive (January–April 2026)

A dedicated campaign is underway from January 15 to April 15, 2026, to ensure all remaining eligible beneficiaries — especially senior citizens — get their Ayushman cards created.

 

Ayushman Bharat Digital Mission (ABDM)

Complementing the physical scheme is the Ayushman Bharat Health Account (ABHA), which creates a unique digital health ID for every citizen. This enables continuity of care across hospitals, stores health records digitally, and ensures that patient data flows seamlessly between primary and tertiary care facilities. It is the technological backbone of India’s digital health ecosystem.
